The All Age Disability Service works alongside people with disabilities of all ages and their carers to support their personal, social care and health outcomes. 

The team also support children and young people with mental health issues.

The team also supports young carers.

The Independent Travel team is also part of the All Age Disability Service. The team supports children and adults to develop skills in travelling independently, road safety and personal safety.

Who works in the All Age Disability Service?

The team includes, Social Workers, Occupational Therapists, Special Education Needs (SEN) Officers, Community Nurses, Speech and Language Therapists, Consultant Psychiatrists and a Psychologist. Some staff work for the council and some staff work for Coventry and Warwickshire Partnership Trust, which is part of the health service (NHS).

There are 2 teams, who work with people of different ages:

• 0-17 years 
• 18+ years

What support can I get and how do I contact the service? 

The service offer information and advice to everyone.

If your child is aged 0-5 you may be eligible for support from the Early Support team.

For some support and services you or your child will need to be eligible for support in line with the Children and Families Act.

If you are aged  0-14, or caring for child or young person of this age you need to need to contact the Referral and Assessment team.

Services can be also be co-ordinated for disabled children through the Common Assessment Framework (CAF).

Support for young people (15-25)

What support can I get and how do I contact the service?

The 15 – 25 team with the All Age Disability Service provide specialist information, advice and support to children and young people with health or social care needs during the transition process.

Transition is the period of time when young people are moving from childhood to adulthood. This is a very important stage in a young person’s life because they need to make plans for their future, including any care and support which will help them live as independently as possible.

Your assessment will be an ongoing process and will be based on what you need, want and hope for as you begin your adult life. It will consider your wellbeing, including building a fulfilling social life and making a contribution to society, as well as your basic living needs.

For some support and services, you or if your child is 17 and under, will need to be eligible for support in line with the Children and Families Act. Services can be also be co-ordinated for disabled children through the Common Assessment Framework (CAF).

If you or the person you care for is aged 18 and over then your assessment will be in line with the Care Act.  Find out more about the assessment process for care and support for adults. and the different types of support that might be available to help you.
