Community Children's Nursing Team
The team supports children with Complex health needs/ specialist nursing needs in their own homes, in school and in respite settings. The team provide nursing and carer support to children in 5 named special schools in Coventry. It also supports children who meet continuing care criteria in their own homes and in school by providing bespoke packages of care. The team delivers end of life care to children which includes in the last few days of life 24 hour visiting as needed.

Access to the service is by referral from a Consultant Paediatrician, Community Paediatrician, Children's Hospital ward or other Allied Health Care Professionals.
If children have been referred to the service but do not have a nursing need they will be signposted to other services that are available.
