Age UK Coventry & Warwickshire - Hospital Social Prescribing

Following a stay in hospital, your health or care needs may have changed.

The Hospital Social Prescribing service aims to help people aged 18+ to achieve a safe and sustainable discharge from hospital. Our Hospital Social Prescribers will spend time with you, to understand your needs and assist with the acquisition of adaptations such as installation of a key safe or a lifeline. Social Prescribers can also signpost or refer into a range of other local support services.

The service operates from Warwickshire hospitals: George Eliot in Nuneaton, Rugby St Cross and South Warwick, and includes surrounding community hospitals. 

How to access the service:

Referral into this service can be made directly through hospital staff and information about the service can be found by emailing or phoning us.
